Purpose of Family Resource Package

The Family Resource Package has been designed for long-term care team
members to share specific resources with individuals living in long-term
care, family members, and carers.

Overview of Family Resource Package

  1. What is the difference between Palliative Care & End of Life (EoL) care? What are the benefits?
  2. Understanding disease progression, and how family and carers can support their loved ones:
          a. Progressive neurological illnesses, including dementia
